  • Certain hazardous, recurring activities performed at Jefferson Lab are restricted and only undertaken when authorized by a temporary work permit. These work permits ensure that all hazards have been considered and associated risks have been acknowledged and mitigated.
  • Jefferson Lab considers no activity to be so urgent or important that its standards for environment, safety, or health (ES&H) may be compromised. Therefore, all individuals at Jefferson Lab have the responsibility, and authority, to stop work when conditions appear to pose a hazard. This stop-work and re-start program is provided to ensure the safety of individuals, equipment, and the environment.
  • Suspend work occurs when a hazard issue is discovered or anticipated that may endanger individuals, equipment, or the environment if the activity continues. Work is suspended until mitigation measures are implemented or a new, safer, work plan is developed.
  • A Stop Work Order is initiated when an imminent danger has developed, a hazard issue is discovered and a Task Hazard Analysis (THA) reveals a post mitigated Risk Code >3, observed activity appears to represent imminent danger, or individuals cannot resolve a hazard issue in an acceptable manner.
  • Instructions:

  1. The Worker’s supervisor/Technical Representative (SOTR)/sponsor fills out this form.  (Document the incident objectively as possible.)
  2. Review required by the individual identifying the Hazard/Imminent Danger. 
  3. Distribute to those listed as soon as possible.
  • Jefferson Lab provides this work re-start procedure to ensure the safety of individuals, equipment, and the environment after a work delay. Characteristics of each delay, including urgency, resources, timeliness of communications, etc, differ. This document provides the minimum requirements to ensure that safe, effective work planning has been enacted prior to work re-start.
